Overview

Trajectory® Healthcare, LLC is developing version 2.0 of its breakthrough patient time^TM based methodologies and decision support tools to objectively describe and improve resource allocation in medical, disease, and population health management.

The Trajectory Patient Time System, v1.0

Trajectory version 1.0 is operated only by Trajectory associates. It is a patent-pending, powerful outcomes-based decision support system designed to improve clinical and financial resource allocation.

In traditional decision support systems, measures of variation in the "natural history" of disease and utilization are obliterated by averaging resource consumption metrics by "member months" (e.g., PMPM).

Trajectory uses the same underlying methods as prospective clinical trials: Enrollment of patients in defined populations and the description, analysis, and trending of individuals within those populations over patient time segments.

Trajectory is a unique approach that uses health data to create defined populations and predict, as populations, their expected course.

These defined populations are made up of individuals who meet specific expert-based criteria for inclusion and are then grouped together based upon the calendar date on which each meets specified criteria. (i.e., Administrative Incidence^TM ).

Trajectory tracks these populations over discrete patient time segments. Longitudinal trends of the "natural history" of disease and utilization are revealed. Stratification of these trends lead to a powerful series of "what if" scenarios from which one or more empirical-based care management solutions emerge. These hypothesized solutions can then be implemented in population sub-groups and tested for clinical and/or financial effectiveness using Trajectory’s objective impact module.

This superior epidemiological-based system merges clinical and financial data leading to actionable and testable management solutions. Trajectory measures exactly what needs to be managed: It's about time^TM.

Trajectory, the more user-friendly version 2.0, will be offered for beta testing in the near future.
